There are many types of mental health professionals with a wide range of expertise, entailing different specialties and subspecialties, all of whom have a particular area of treatment focus and treatment philosophy.

-As such, my screening/ intake process is designed specifically to help me better understand an individual's particular circumstances.

-It's important to elaborate thoroughly while completing the questionnaire, so that I have the full scope of understanding regarding one's history as well as present concerns.

After you contact me by e-mail, the intake questionnaire will be e-mailed to you.

-The subject of the e-mail will be as follows: "Dr. Alamgir has sent you an Intake Form"

-Typically the most effective way to find this e-mail is to utilize the search function of your e-mail by searching "IntakeQ"

-If you don't see the e-mail in your inbox, you may need to allow more time until the e-mail is delivered or it might be in your junk/ spam folder.

-Once you have completed the questionnaire, you can electronically submit online (without need for attachments, printer, scanner, fax machine).

The information below summarizes some of the information on my website and frequently asked questions from prospective patients.

I treat patients in Texas ages 18-60.

I have an office in Dallas.

-Due to COVID-19 pandemic precautions, I'm only conducting online appointments at this time.

-Online appointments require reliable internet connection and a device with audio and video capabilities (including smartphone, laptop, tablet, or desktop computer).

Appointments are available on both weekdays and weekends.

-Same day appointments are available.

Regarding appointment fees,

-Initial consultation is $200

-Follow up is $125

-method of payment: credit card, debit card, HSA, FSA, ACH electronic bank transfer

-We do not accept/ process claims for any insurance, Medicare, Medicaid, etc.

-However, if you have insurance, upon your request, we can provide you with a "superbill" (essentially a receipt with diagnostic codes) that you can directly submit to your insurance provider (for reimbursement that generally is offered through out-of-network insurance benefits).
